These services include the processing of test requests, patient preparation and identification, specimen collection, transport, and storage, as well as the processing, analysis, validation, interpretation, reporting, and recommendations regarding clinical specimens.

Laboratory test results have a significant impact on clinical decisions. Ideally, regardless of where they are performed, it is desirable to have a system in place that ensures test results are consistent, accurate, and reliable worldwide and that monitors compliance with these standards. This is only possible if medical laboratories are included in accreditation systems that adhere to specific standards.

In our country, medical laboratories require special conditions in terms of the needs and expectations of patients and clinical staff; They establish accreditation requirements in accordance with ISO 15189, the standard titled “Specific Requirements for the Quality and Competence of Medical Laboratories,” which is based on ISO/IEC 17025 and ISO 9001.

The high professional and technical standards of medical laboratories and the quality of the services they provide are issues that both the public and individual patients frequently prioritize and monitor. To this end, medical laboratories—like other businesses—have focused on continuous improvement initiatives. Unlike other businesses, however, issues such as patient safety and confidentiality take center stage in medical laboratories. Medical laboratories are committed to demonstrating their competence in their respective service areas through a review and evaluation process based on ISO 15189—the most appropriate standard—and to publicly announcing their compliance with this standard.

The primary responsibility of a medical laboratory undergoing the accreditation process is to ensure its technical competence by properly carrying out each stage of the process: the pre-analytical phase, the analytical phase, and the post-analytical phase.

In studies related to medical laboratories, it has been determined that a large proportion of inconsistent results regarding patients’ clinical conditions stem from errors occurring during the preanalytical phase. To minimize the risk of errors occurring during this phase, it is essential to ensure proper preparation before collecting samples from patients—for example, collecting and storing samples under appropriate conditions and ensuring that the transfer of samples, which is of critical importance, takes place under suitable conditions. Within the framework of the quality assurance system and accreditation, temperature control and monitoring of the transport bags used during shipment can be ensured using data loggers. After all these sample-related procedures, the registration of the sample upon arrival at the laboratory must also be carried out in accordance with the principles of confidentiality and reliability. Thanks to the information management and barcoding systems used for this purpose, no confusion and/or loss or alteration of information occurs in the records.

The most important stage of the analytical process is, in practice, the stage at which the analyses are performed. Thanks to the systems used during sample registration, the location where the required tests for each sample will be performed is automatically recorded, and samples are sorted according to their work areas. In this stage, internal quality control activities—one of the accreditation requirements—are performed daily and recorded. This ensures the traceability of inter-day variations and analytical performance, as well as the detection of any potential nonconformities. If a nonconforming result is obtained, analysis is suspended until the nonconformity is resolved. After the necessary corrections are made and corrective actions are taken, analysis of patient samples may resume. External quality control studies are an indispensable part of the accreditation process. These are conducted at regular intervals with broad participation through proficiency testing providers such as RIQAS, EQAS, KBUDEK, and others. For tests without an external quality control program and/or when necessary, interlaboratory comparison (ILC) tests may be organized to assess compliance. All of these quality control activities ensure the reliability of analytical results and guarantee method quality through continuous monitoring of the methods.

After the required tests on the samples have been performed, the results are transferred via the same system to the relevant area for reporting. The rejection of non-compliant samples and notification of the appropriate personnel are carried out by the sample acceptance/service departments. Following the analysis, the results recorded in the system by the relevant technical staff and physicians are analyzed from both an analytical and medical perspective and reported. Reports must be backed up via the system and server and retained for the specified periods, based on the principle of ensuring the security of the records. In addition, test samples must be stored under appropriate conditions for periods appropriate to the type of analysis, in case of potential repeat analyses and/or requests for new tests.

The concept of technical competence is quite comprehensive and includes criteria such as personnel competence, the adequacy of testing and analysis methods, and the suitability of equipment. This process primarily involves the selection and training of appropriate personnel, the selection of testing and analysis methods suitable for the task at hand—and ensuring they are adaptable to future developments when necessary— the use of equipment appropriate for the selected methods and the calibration of such equipment, ensuring the reliability of the analysis and/or the analyst, as well as the establishment and monitoring of infrastructure such as environmental and facility conditions.

Accreditation is entirely voluntary. The cornerstones of accreditation are competence, impartiality, and independence. Accreditation and quality practices are a long-term journey undertaken to be implemented throughout one’s career. In addition to benefits such as improving the laboratory’s service quality, reducing costs, and demonstrating reliability, accreditation ensures credibility and acceptance on international platforms. By securing recognition from third parties, it ensures that the laboratory undergoes only a single audit, thereby preventing the need for multiple audits. Through the continuous training and development of laboratory staff, a competent workforce is established that operates professionally within the industry. Furthermore, by keeping pace with evolving technology—including the renewal of instruments and equipment and the refinement of existing methods—laboratories are positioned to effectively lead in their areas of expertise. As a result of all this, patient and family confidence and satisfaction increase, it has a positive impact on capacity growth, and it enhances the laboratory’s reputation and prestige.

Successful quality improvement efforts in medical laboratories may be achieved by encouraging the establishment of ongoing organizations dedicated to reducing errors, improving patient safety, and enhancing the quality of care through reports and indicators related to the development of care quality. All of these can lead to improvement through a revision of generally accepted practices in the healthcare field. To bring about behavioral change, various methods can be implemented, such as ongoing and effective training, feedback, participation, administrative rules, financial incentives, and/or penalties. Performance reports that provide realistic performance criteria and risk-adjusted outcomes can help improve laboratory quality.

Reference:

https://indigodergisi.com/2014/12/laboratuvarda-neden-akreditasyon/

WHY IS MEDICAL LABORATORY ACCREDITATION NECESSARY? “At our laboratory, which is part of the TÜRKAK TS EN ISO 15189 accreditation system, we provide fast, accurate, and reliable test results.” Medical laboratory services are essential for patient care and must meet the needs and expectations of all patients and all clinical staff responsible for their care….

[125

post-13960","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-blog
["GET
